Overview
Intervertebral Disc Disease (IVDD) is one of the most serious spinal conditions affecting dogs, particularly certain breeds with genetic predispositions. This neurological disorder occurs when the cushioning discs between the vertebrae in the spine either rupture (herniate) or bulge, putting pressure on the spinal cord and nerves. The condition can range from mild discomfort to complete paralysis, making early recognition and treatment crucial for the best possible outcome.
IVDD primarily affects the cervical (neck) and thoracolumbar (mid to lower back) regions of the spine. The disease is classified into two main types: Type I, which involves sudden disc rupture and is more common in younger dogs, and Type II, which involves gradual disc degeneration and typically affects older dogs. Both types can cause significant pain and neurological deficits, but Type I IVDD often presents as a medical emergency requiring immediate intervention.
The severity of IVDD is graded on a scale from 1 to 5, with Grade 1 representing mild pain and Grade 5 indicating complete paralysis with loss of deep pain sensation. Understanding these grades helps pet owners recognize the urgency of their dog's condition and seek appropriate veterinary care.
Causes
IVDD develops due to a combination of genetic factors and mechanical stress on the spine. Diagnosis
Veterinary diagnosis of IVDD involves a comprehensive neurological examination and advanced imaging techniques. The veterinarian will assess the dog's gait, reflexes, pain responses, and overall neurological function to determine the location and severity of the disc problem.
Neurological examination includes testing deep pain sensation, which is crucial for determining prognosis. The veterinarian will pinch the dog's toes and look for a conscious response, as loss of deep pain sensation indicates severe spinal cord damage and requires emergency surgery.
X-rays (radiographs) can reveal changes in disc spaces and vertebral alignment but cannot directly visualize the spinal cord or herniated disc material. However, they help rule out other conditions like fractures or tumors and can show characteristic changes associated with IVDD.
Advanced imaging techniques provide definitive diagnosis. Magnetic Resonance Imaging (MRI) is the gold standard for diagnosing IVDD, as it clearly shows the spinal cord, disc material, and surrounding tissues. Computed Tomography (CT) with myelography, where contrast dye is injected around the spinal cord, is an alternative when MRI is not available.
Treatment Options
Veterinary Treatment
Treatment approach depends on the severity of symptoms and neurological grade. Conservative medical management includes strict cage rest for 4-6 weeks, anti-inflammatory medications, pain relievers, and muscle relaxants. This approach is typically reserved for Grade 1-2 cases with mild symptoms.
Corticosteroids may be prescribed to reduce spinal cord inflammation, though their use is controversial due to potential side effects. Newer anti-inflammatory drugs and nerve pain medications are increasingly used as safer alternatives.
Physical therapy and rehabilitation play important roles in recovery. Controlled exercises, hydrotherapy, laser therapy, and acupuncture can help improve mobility and reduce pain during the healing process.
Surgical Treatment
Emergency surgery is often necessary for Grade 4-5 cases or when conservative treatment fails. The most common procedures include hemilaminectomy, where part of the vertebral bone is removed to access and remove herniated disc material, and ventral slot surgery for cervical IVDD.
Fenestration involves removing the center of multiple discs to prevent future herniations and may be performed alongside decompressive surgery. The goal is to relieve pressure on the spinal cord and prevent further damage.
Post-surgical care requires intensive monitoring, pain management, and gradual rehabilitation. Many dogs need assistance with urination and defecation initially, and physical therapy is essential for optimal recovery.
Home Care
Strict rest is the cornerstone of conservative treatment and post-surgical recovery. Dogs must be confined to a small area or crate, with leash walks only for bathroom breaks. Jumping, running, and stair climbing must be completely avoided during the healing period.
Pain management at home involves administering prescribed medications exactly as directed. Never give human pain medications, as many are toxic to dogs. Monitor for side effects like vomiting, diarrhea, or changes in appetite.
Assist with mobility as needed, using slings or harnesses to support weak limbs. Provide soft, supportive bedding and ensure easy access to food and water. Some dogs may need help with urination and defecation, requiring manual expression of the bladder.
Prevention
Weight management is crucial for preventing IVDD, as excess weight puts additional stress on the spine. Maintain dogs at an ideal body condition through proper diet and controlled exercise appropriate for their breed and age.
Minimize high-impact activities and jumping, especially in predisposed breeds. Use ramps instead of allowing dogs to jump on and off furniture or into vehicles. Avoid activities that involve sudden stops, sharp turns, or excessive spinal flexion.
Regular, moderate exercise helps maintain muscle strength and spinal flexibility. Swimming is an excellent low-impact exercise for dogs prone to back problems. Avoid overexertion and gradually increase activity levels.
Environmental modifications can reduce injury risk. Install baby gates to prevent access to stairs, provide orthopedic bedding for joint support, and ensure safe, non-slip surfaces throughout the home.
Prognosis & Recovery
The prognosis for IVDD varies significantly based on the severity of symptoms and speed of treatment. Dogs with mild symptoms (Grade 1-2) who receive prompt treatment have excellent prognoses, with most returning to normal activity within 6-8 weeks.
Moderate cases (Grade 3) have good prognoses with appropriate treatment, though some dogs may have residual weakness or gait abnormalities. Recovery typically takes 2-3 months with proper rehabilitation.
Severe cases (Grade 4-5) have more guarded prognoses, particularly if deep pain sensation is lost. However, with emergency surgery and intensive rehabilitation, many dogs can regain function, though recovery may take 6 months or longer.
Factors affecting prognosis include the speed of symptom onset, duration before treatment, age of the dog, and presence of deep pain sensation. Dogs who lose deep pain sensation have only a 50-60% chance of regaining function even with surgery.
When to See a Vet
Immediate veterinary attention is required if dogs show any signs of paralysis, loss of bladder or bowel control, or inability to feel pain in their limbs. These symptoms indicate potential permanent spinal cord damage if not treated within hours.
Seek prompt veterinary care for persistent back or neck pain, reluctance to move, changes in gait, or any neurological symptoms. Early intervention significantly improves outcomes and may prevent progression to more severe grades.
